Trebuie s\u0103 completa\u021bi acest formular \u00een \u00eentregime \u0219i s\u0103 \u00eel semna\u021bi. Formularul de \u00eenregistrare poate fi transmis prin intermediul unei fotografii sau poate fi predat la cabinet. Pe l\u00e2ng\u0103 formular, avem nevoie \u0219i de o copie a c\u0103r\u021bii de identitate, a documentului sau a pa\u0219aportului dumneavoastr\u0103.
